Job Summary:
We are seeking a Nurse Practitioner to join our Hospice team. The ideal candidate will have experience in hospice care and be able to perform Face-to-Face (F2F) encounters with patients.
The successful candidate will report directly to the Executive Director or Administrator and be responsible for gathering clinical information to support the hospice diagnosis and prognosis.
Key Responsibilities:
- Perform F2F visits with patients
- Gather clinical information to support hospice diagnosis and prognosis
- Communicate findings to the hospice physician
About You:
To be considered for this role, you must have a Master's or higher degree in Nursing and current state licenses as a registered nurse and advanced practice nurse.
You should also have national certification as a Nurse Practitioner and a collaborative agreement in place.
