Hospice Nurse Practitioners play a critical role in delivering high-quality patient care. As a member of our team, you will be responsible for gathering clinical information, performing Face-to-Face encounters, and communicating findings to physicians responsible for certifying or recertifying patients for hospice eligibility.

About You:
To succeed in this role, you must possess a Master's or higher degree in Nursing, completion of a graduate-level nurse practitioner education program accredited by a national accrediting body, current state license as a registered nurse, and current national certification as a Nurse Practitioner (ANCC or AANP). Additionally, you must have a collaborative agreement in place.

- Perform physical assessments for F2F encounters within regulatory time frames
- Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists
- Makes appropriate recommendations to IDGs based on findings of F2F assessments
- Notify hospice agency when unable to complete a F2F visit according to schedule
