We are looking for a Personal Care Outreach Coordinator to join our team. This position will directly report to the Assistant Director PC or Branch Director PC and is responsible for overseeing client referrals, scheduling and performing assessments and marketing to referral sources.

- Responsible for overseeing VA and Private Duty client referrals
- Assists Veterans and families on navigating the VA system and LTC Insurance
- Performs internal and external marketing
- Completes weekly statistical report pertaining to intake and discharges
- High school graduate, some college preferred
- 2-4 years’ experience in sales, marketing, customer service preferred
- Prior experience in healthcare
- Must have excellent professional communication and organizational skills. Bilingual preferred (Spanish)
