Responsibilities:
- Perform face-to-face encounters with patients to gather clinical information.
- Communicate findings to the certifying physician.
- Work collaboratively with the IDG to make recommendations.
Requirements:
- Masters or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- A collaborative agreement in place.
