CKY Capacity Management Coordinator (Weekends) Essential Functions Determines patient priority and the ability to transfer patients between units and hospitals when requested beds exceed available beds. Acts as a liaison for physicians and hospitals to facilitate the transfer of patients into the institution Monitors workflow between the market and the transfer center Uses advanced clinical knowledge to direct proper patient placement and utilization of beds in accordance with established standards and policies. Utilizes advanced theory to provide transfer and customer satisfaction. Communicates patients needs through appropriate verbal and written communications. Attends educational in-services as appropriate and participates in professional activities and organizations to maintain knowledge of current trends, practices and developments. Collects and analyzes data from various hospital departments to appropriately determine patient placement. Regularly reviews census and acuity information to determine or anticipate potential problems or trends and to prevent the disruption of services. Serves as a resource to the transfer center and department Directors on bed status of hospital patient care unites and on placement of particular patients. Supports and participates in research and related activities as needed. Helps identify problems and develops solutions and protocols to improve the quality of patient care. Some case management responsibilites as needed : - Completes discharge follow up calls and coordinates care for identified needs. - Performs utilization review of patients admitted overnight to ensure appropriate inpatient or observation status.
Qualifications
Job Requirements Minimum EducationAssociate’s Degree Nursing (ADN) RequiredBachelor’s Degree Nursing (BSN) Preferred Required SkillsLicenses : Registered Nurse in State of KY Three years of recent (within the past 2 years) nursing experience that includes supervision and patient bed placement.BLS with in 30 days of hire Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
