SUMMARY
Community Living Managers will provide an array of services which are necessary to support persons with intellectual/developmental disabilities to live in the community. Engage in resource development for the individuals served and ensures their life goals are implemented, personal outcomes are achieved, and the quality of care is high in every respect. The Community Living Manager assists in problem solving, communicating regularly with our individuals, families and Associate Directors. They provide supervision for the Direct Support Professionals (DSP).
ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following. Other duties may be assigned.
Ensures CLAs operate in compliance with Healthcare Facilities Regulations, local, state and federal rules and regulations including HIPP and other governing bodies.
Ensures staff and homes operate in compliance with InCommunitys policies and procedures, specifically in the areas of behavior management techniques, handling emergency situations and medical situations.
Assisting in recruiting, training, scheduling, supervising, and conducting performance evaluations of assigned staff.
Train and supervise DSPs for success. Conduct interviews for hiring staff.
Schedule and facilitate regular household meetings quarterly with staff and individuals, assisting in problem solving as needed and celebrating accomplishments. Ensure staff is focused and well trained on ISP goals and personal outcome measures.
Provide supervision of assigned residential home(s) to ensure the health, safety, needs and desires are met
Provide supervision to DSPs regularly. Conduct annual professional development and evaluations of DSPs, intermittently fill in for vacancies in case of an emergency - On-call for emergencies for individual issues.
Attend meetings and training events to continuously upgrade skills and knowledge of state-of-the-art concepts, particularly personal outcome measures.
Ensures homes remain in compliance with budgets (staffing, food, household supplies, etc.) Participates in budget reviews on a quarterly/monthly basis.
Build relationships with the families of the individuals in the homes; provide updates
Maintains accurate and complete records of all Individuals personal spending monies in accordance with rules and regulations; completes monthly reconciliation, submit receipts and paperwork to Administrative Assistant by the 5th of every month.
Maintains accurate and complete records of house spending (including credit card purchases and petty cash purchases) in accordance with rules and regulations; completes monthly reconciliation, submit receipts and paperwork to Administrative Assistant by the 5th of every month.
Ensures van binder is in compliance and monthly mileage is tracked.
Utilizing site visit tool, inspects home to ensure that home meets all safety requirements, checking for clutter and dangerous objects. This is done monthly
Ensures evacuation drills, both fire and severe weather; records result of each drill are done per standards and licensure requirements
Notifies supervisor of all medical emergencies as outlined by policies and procedures, including submission of GERs.
Ensures MY25 menus are followed and posted weekly in accordance with regulations and previous three months menus are filed in the home in the homes.
Ensure adequate food supply is in home, in accordance with approved menus.
Maintains objectivity in position to set appropriate limits while working with consumers.
Completion of a person-centered calendar each month which supports the implementation of activities as designed by the individuals support team. Ensures activities support individuals ISP goals.
Coordinates transportation and supervision for residents as dictated by individual program.
Monitors staff documentation: Activity Notes, ISP Tracking, Behavioral Support Plans, and House Logs and ensures that all necessary documents are accurate and person centered.
Approves employee time cards and manages overtime
Ensure all necessary documentation from appointments are on-site.
Assure staff is aware of medication changes/treatment changes.
Responsible for monthly staffing schedules and ensures the homes stays within allocated hours.
Ensures that each individuals medications are received monthly and reviews Medication Administration Reports for assigned homes.
Ensures and monitors homes for cleanliness, furniture is good repair, food supply and ensures that ISP are current for each individual.
Ensure staff and associate directors are aware of medication changes/treatment changes.
Communicates professionally and treat the people we serve, parents, service monitors/coordinators, and InCommunity employees with respect.
Notify supervisor of problem areas needing counseling or disciplinary action for subordinate staff/document same.
Adhere to InCommunitys policies concerning confidentiality of consumer information.
Adhere to InCommunitys documentation standards for participant and provider records.
Ensures that all mail is turned into the main office in a timely manner.
Monitors the vehicles assigned to the homes i.e. cleanliness, good repair etc.
Qualifications
QUALIFICATIONS: To perform this job successfully, an individual must have acute hearing and speech skills to communicate with the public, family members, and outside agency program and management staff. Must have physical stamina to sustain occasional intense periods of activity and stress. Must be able to lift 50 pounds.
EDUCATION and/or EXPERIENCE: High School Diploma and a minimum of 2 years coordination experience in the area of intellectual/developmental disabilities, preferably with the focus being in community-based programs or Bachelor Degree with a major in social services and 1-year experience.
CERTIFICATES, LICENSES, REGISTRATIONS
CNA preferred.
LANGUAGE SKILLS
Ability to read, ability to write and effectively present information and respond to questions from groups of managers, individuals, and the general public.
MATHEMATICAL SKILLS
Ability to manage and track individuals funds and report to the Finance Department.
EMOTIONAL INTELLIGENCE
The employee must demonstrate and maintain emotional intelligence as a supportive team member performing all the essential functions and duties of the position while managing the stresses associated with personal domestic life and those stresses associated with serving individuals with disabilities.
PHYSICAL DEMANDS
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
