SUMMARY
Wellness Coordinators will attend medical appointments and provide coordination of medical aspects of assigned case load; fulfills wellness, both physical and mental health, role of Support team, conduct at minimum monthly onsite monitoring.
ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following. Other duties may be assigned.
Schedule, Transport and coordinate individuals medical visits and ensure the documentation required at visits is completed; collect and monitor all medical visit forms/documentation (review), including follow up data.
Notify Community Living Managers of any new diagnoses for the individuals and provide any educational material (if applicable).
Ensure all medical visits summaries are uploaded in Therap and all appointments entered in Therap.
Ensure all prescriptions and orders are faxed to the pharmacy after all appointments and are in QuickMAR.
Enter all OTC medications in QuickMAR and updated annually.
Ensure medication transfer is complete for any new individuals or transfer, transition of individuals, internally and externally.
Ensures staff and homes operate in compliance with InCommunitys policies and procedures, specifically in the areas of behavior management techniques, handling emergency situations and medical situations.
Ensure individuals profile in QuickMAR is completed and updated as needed.
Audit QuickMAR monthly to ensure all RX are present and send communication to supervisor regarding missing prescriptions.
Upload healthcare plans in QuickMAR as needed.
Review HRST Data Tracking Log Monthly and enter weights in QuickMAR monthly.
Monitor allergies and dietary constraints/prescriptions for accurate and appropriate implementation as well as correct information is reflecting on all internally and externally generated documentation (onsite medical information book)
Monitor and provide general oversight of bowel tracking, blood sugar monitoring, and blood pressure monitoring, seizure tracking, skin integrity and shift change) via QuickMAR and Therap accurate reporting and following of protocols.
Monitor the need for or the receipt of lab work; collect data and ensure proper follow up if required. Upload labs in Therap.
Review MARs weekly to ensure MARS are accurate and there arent any errors on MAR. Follow up with Community Living Managers (CLM) regarding any errors.
Follow up on ER visits and hospitalizations to ensure all paperwork is received and all follow up appointments scheduled, all orders received.
Ensure individual rights are appropriately maintained; report violations.
Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences.
Remain in active communication with supervisor on all program matters.
Qualifications
QUALIFICATIONS: To perform this job successfully, an individual must have acute hearing and speech skills to communicate with the public, family members, and outside agency program and management staff. Must have physical stamina to sustain occasional intense periods of activity and stress. Must be able to lift 50 pounds.
EDUCATION and/or EXPERIENCE: High School Diploma and a minimum of I/DD consumers or medical field, one of which must be in a direct support role or Associates Degree in Social Service field; or certified CNA or MOA; Experience may be substituted for education with Director of Residential Services approval.
CERTIFICATES, LICENSES, REGISTRATIONS
CNA preferred.

Ability to read, ability to write and effectively present information and respond to questions from groups of managers, individuals, and the general public.
