Job Description
This is an exciting opportunity for a skilled social worker to join our hospice care team. As a Social Services Coordinator, you will be responsible for psychosocial evaluations, ongoing counsel, bereavement services, and community education outreach.
Working in accordance with the plan of care, you will provide physical, emotional, and spiritual support to patients and families when it's needed most. Your expertise will enable you to create personalized plans that cater to each patient's unique needs.
- Develop and implement effective psychosocial evaluations to identify patient needs
- Provide ongoing counsel and support to patients and their families
- Offer bereavement services to help patients cope with loss
- Promote community education and outreach to raise awareness about hospice care
Requirements
To be successful in this role, you will require:
- A Master's degree in Social Work from an accredited institution
- At least 2 years of clinical work experience, preferably in healthcare or hospice
- The ability to sit, stand, bend, move intermittently, and lift at least 25 pounds
