Job Description
We are looking for a skilled Community Outreach Representative to join our team at Georgia Hospice Care. The successful candidate will be responsible for coordinating new hospice business in existing and new accounts, identifying new markets, and maintaining existing service.
This position involves daily interaction with patients, medical professionals, other referral sources, and the community to assure continuity of care and to coordinate appropriate communication and documentation.
The ideal candidate will have excellent communication skills and the ability to build strong relationships with referral sources and healthcare providers.
