* Create personalized care plans for patients based on their unique needs and circumstances
* Collaborate with patients, families, and the care team to ensure seamless communication and effective care delivery
* Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements
* A heart to serve patients and families and a passion for providing the best possible care
* Graduate of an accredited school of nursing with a current state license as a registered nurse
* Experience: 2 years of nursing experience in a clinical care setting (hospice experience preferred)
* Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
