Job Responsibilities

As a Patient Support Worker, you'll play a vital role in delivering holistic care to our patients. Your responsibilities will include:

• Conducting thorough psychosocial evaluations to identify patient needs
• Developing personalized plans that address the physical, emotional, and spiritual needs of patients
• Providing ongoing support and counseling to patients and their families
• Promoting community education and outreach to raise awareness about hospice care

Qualifications

To be successful in this role, you'll require:

• A Master's degree in Social Work from an accredited institution
• At least 2 years of clinical work experience, preferably in healthcare or hospice
• The ability to sit, stand, bend, move intermittently, and lift at least 25 pounds
