Clinical Case Manager
Job Summary:
Georgia Mountains Health is a Federally Qualified Health Care (FQHC), Primary Care facility that is mission driven to provide access to care by providing professional, compassionate and affordable, quality healthcare to our communities. We are looking for a Clinical Case Worker for an immediate full-time position in the Chatsworth location, work schedule of 8a-5p, M-F, no nights, no weekends and holidays off.
The Case Manager will be part of the primary care team resolving patient issues that could affect smooth care progression in regard to providing coordination of patient care with external service resources to address their medical and social determinants of health needs. Some key responsibilities include but not limited to:
- Community networking to connect patients with essential resources
- Referral coordination
- Insurance enrollment
- Coordinating information, gathering and reporting daily and weekly patient statics
- Providing education to others to ensure understanding and effective communication.
This position requires excellent customer service skills and a working knowledge of medical terminology and medical office procedures. Experience with electronic medical records, Athena preferred. Experienced Medical Assistants are encouraged to apply, experience in primary care/family health experience highly preferred.

Requirements:
- 1-2 years experience in the field of social work/case management or similar field.
- Medical Assistant preferred
- Ability to speak Spanish highly desired.
- Satisfactory background check and drug test required.
