We are seeking a Home Care Case Manager As a Case Manager, you will play a crucial role in ensuring the highest level of care and service to our clients. The ideal candidate is empathetic, highly organized, and possesses excellent customer service skills.
Responsibilities
Maintaining a fully staffed caseload at all times
- Meeting all client needs and ensuring caregivers are appropriately matched and trained
- Updating your supervisor on client birthdays within your caseload so a gift can be arranged
- Ensuring client compliance with care plans, care assessments, and all necessary reports
- Conducting 24-hour check-in calls following the start of service
- Conducting a 10-day courtesy call with case managers after service initiation
- Completing 30, 60, and 90-day reports for your clients
- Performing bi-monthly home visits for quality assurance
- Leading the initial intake process for new clients assigned to you
- Ensuring care plans are updated annually for each client
- Qualifications
- High school diploma or GED
- Previous experience as a Home Care Case Manager or in a similar position is preferred
- Highly organized with attention to detail
- Comfortable with Microsoft Office and other computer programs
- Ability to multitask and prioritize projects
- Excellent customer service and interpersonal skills
