Nurse Practitioner Job Description
Looking to join a team of compassionate providers with an emphasis on the continuity of care model? Geriatric Primary Care is seeking a compassionate, dedicated Nurse Practitioner to join our team in the Wichita area! This role has the potential for growth for the right candidate. Previous experience with Geriatrics is a plus but not required. The role will provide primary care within an assisted living and independent living setting. This position will allow for both collaboration and autonomy as a Nurse Practitioner.
We provide Primary Care Services to assisted living, life plan communities, skilled nursing and long-term care facilities. Our care model is unique, giving our providers the ability to focus on patient care with a robust technology platform allowing for remote patient monitoring, patient scheduling, along with support staff to empower our team. Qualifications
· Licensed and Board certified as an adult or family nurse practitioner
· Licensed in Kansas
· Federal DEA/license eligibility
· Must provide quality and timely documentation
· Must be an effective oral and written communicator, with outstanding clinical skills, high character, good judgement, with customer focus and an execution mindset. We want clinical leaders!
· Must possess good collaboration, organizational, and punctual skills

Responsibilities
· Establish and maintain trust with the patient/families to promote understanding and assist them in making informed decisions
· Obtain and review medical history, analyze and diagnose conditions and develop an appropriate plan of care
· Interpret and order diagnostic tests/reports and refer appropriately
· Solve complex problems, develop innovative solutions while identifying risk factors and help mitigate barriers to care
· Conduct advanced care planning discussions and trajectory of chronic disease with patient and family members
· Serve as a resource for cost-effective care and medication management
· Effective and timely communication and care coordination with all team members including facility staff, specialists, social workers, therapists, direct care professionals and direct supervisor
· Attend virtual monthly/bi-monthly team meetings/trainings and promote on-going advancement of clinical expertise and knowledge through continuing education

Pay: $36,000.00 - $100,000.00 per year
