Key Responsibilities
- Schedule and perform in-home assessments to gauge nursing facility or institutional level of care requirements.
- Review level-of-care (LOC) documentation and medical records to validate eligibility for program participation.
- Educate parents and guardians on essential care procedures or organize suitable educational services.
- Create individualized care plans and collaborate with healthcare providers for approval and modifications.
- Work alongside Care Advocates to secure approved services for families.
- Maintain consistent communication with families and healthcare providers to ensure seamless care transitions.
- Develop disaster plans and evaluate the necessity for ancillary, specialty, and community services.
- Address any concerns or grievances to guarantee quality care and client satisfaction.
- Minimum of 3 years of recent clinical pediatric experience (required).
- Experience with Medicaid programs is preferred.
- Bachelor's degree in a health-related field is strongly preferred.
- Proficiency in Spanish is preferred.
- Excellent organizational, communication, and multitasking abilities.
- Capability to work independently and manage various responsibilities.
- A valid driver's license and automobile insurance are required.
- Ability to pass a background check and provide an annual negative TB test.
- Current unrestricted RN license in the relevant state of practice.
- Basic Life Support (BLS) certification.
- Certified Case Manager (CCM) certification must be obtained within four years of employment (or another URAC-approved certification).
