Key Responsibilities
- Collaborate with primary care providers to oversee a designated panel of chronic care patients.
- Utilize care management platforms to effectively coordinate and monitor patient progress.
- Identify and address unmet patient needs with timely interventions.
- Empower patients to take an active role in their health management.
- Maintain accurate and organized patient registries to ensure comprehensive health data management.
- Work alongside healthcare teams to optimize care coordination and improve overall patient outcomes.
- Contribute to initiatives aimed at quality improvement and population health strategies.
- Provide professional and courteous interactions with patients through phone and digital communication.
- Ensure adherence to all relevant healthcare policies and regulations.
- Participate in team meetings and necessary training sessions as required.
- Active LPN license with experience ranging from 1.5 to 12 years (experience in population health preferred).
- Strong problem-solving abilities and capacity to multitask effectively.
- Exceptional written and verbal communication skills.
- Excellent customer service and interpersonal capabilities.
- Familiarity with Electronic Health Records (EHR) and Electronic Medical Records (EMR) systems is advantageous.
- Bilingual proficiency in English and Spanish is a plus but not required.
- Licensed Practical Nurse (LPN) certification is mandatory.
- Relevant experience in chronic care management is preferred.

- The salary for this position is fixed at $22 per hour (non-negotiable).
