Key Responsibilities
- Deliver education to patients and caregivers regarding diagnoses, medications, infection control, and safety measures in accordance with the care plan.
- Act as the administrative on-call contact after hours and on weekends, including participation in weekend holiday rotations.
- Oversee weekend management, productivity, and admissions, ensuring effective workflow support.
- Promote collaboration and effective communication with other healthcare professionals to enhance patient outcomes.
- Ensure compliance with relevant state, local, and payer regulations, making sure all documentation aligns with the care provided and meets regulatory standards.
- Supervise and guide weekend clinical staff to ensure adherence to best practices.
- Recognize and report changes in patient status, ensuring timely and accurate documentation that conforms to agency and regulatory requirements.
- Maintain confidentiality of patient information, following all patient rights and agency policies.
- Attend and contribute to staff meetings, patient care conferences, and in-services as required.
- Perform additional duties as assigned, all within the scope of practice.
- Strong clinical skills and a professional demeanor.
- Excellent written, oral, and interpersonal communication abilities.
- Strong organizational and time management skills with meticulous attention to detail.

- Degree in Nursing from an accredited institution.
- Valid nursing license in the state of practice.
- Current CPR certification.
- Previous experience in home care is preferred.
