We are looking for a passionate and dedicated Remote Bilingual Care Coordinator to join our esteemed team and positively influence the lives of patients. In this essential role, you will provide exceptional patient care coordination services, particularly to a diverse group of individuals managing chronic health conditions. Your efforts will be crucial in connecting patients with the necessary resources, ensuring they receive consistent and personalized support throughout their healthcare journey.
Key Responsibilities
- Conduct monthly care coordination calls with patients, healthcare providers, and pharmacies to facilitate continuous patient care between appointments.
- Develop and refine personalized care plans in collaboration with medical practices and healthcare professionals.
- Identify and address social determinants of health and care gaps, ensuring patients receive appropriate referrals and services.
- Empower patients to proactively manage their health and well-being by offering clear guidance within their care plans.
- Make 30-50 calls daily, reaching a minimum of 15 patients to deliver 20 minutes of dedicated care coordination services.
- Ensure all communications uphold patient privacy and confidentiality, adhering to relevant healthcare regulations.
- Minimum of 1 year of experience as a Certified or Registered Medical Assistant (CMA/RMA).
- Bilingual fluency in Spanish and English, with strong verbal and written communication skills.
- Proficient in working with various EMR/EHR systems, Microsoft Office, and Google Suite.
- Ability to think critically and engage patients with professionalism and empathy.
- Self-directed and capable of working independently in a dynamic virtual environment.
- Experience in call centers, triage, or care coordination is preferable.
- Familiarity with geriatric populations or case management is advantageous.
- Certification as a Medical Assistant (CMA or RMA) is required.
- A minimum of 1 year of relevant experience in care coordination or similar roles is necessary.
