Key Responsibilities

As a Care Manager, you will:

• Conduct thorough assessments to fully understand the unique needs, preferences, and aspirations of individuals.
• Develop and uphold customized Life Plans that encompass both clinical and non-clinical supports.
• Coordinate essential services including healthcare, social services, and community resources.
• Facilitate access to chronic disease management, wellness programs, and long-term support services.
• Collaborate with families and support networks to foster a person-centered approach to care.
• Encourage self-advocacy and independent decision-making among individuals.
• Utilize Health Information Technology systems for the documentation and coordination of care.
• Advocate for culturally and linguistically suitable services for every individual.

Qualifications

We seek candidates who meet the following educational and experiential criteria:

• A Bachelor's degree coupled with a minimum of 2 years of relevant experience, OR
• A Registered Nurse (RN) license with at least 2 years of experience, OR
• A Master's degree with a minimum of 1 year of relevant experience.
• Experience working with individuals with intellectual/developmental disabilities (I/DD) is essential.
