Key Responsibilities

• Conduct thorough assessments to identify the clinical, psychosocial, and service requirements of individuals.
• Formulate, execute, and revise person-centered Life Plans that encompass both clinical and non-clinical healthcare needs.
• Offer health promotion and assistance to enable individuals to sustain optimal health and overall wellbeing.
• Oversee chronic disease management and ensure access to essential care.
• Facilitate referrals to clinical and community resources, including planning and follow-up for transitional care.
• Collaborate with an interdisciplinary team to address all needs in a comprehensive and integrated manner.
• Engage with families and natural supports to cultivate a collaborative care process.
• Advocate for individuals, fostering self-advocacy and independence.
• Ensure services are culturally and linguistically suitable.
• Utilize Health Information Technology (HIT) for documentation and service coordination.
• Uphold confidentiality and comply with HIPAA regulations.

Qualifications

• Bachelor's degree with two years of relevant experience OR
• A valid license as a Registered Nurse (RN) with two years of relevant experience (including but not limited to case management/service coordination) OR
• A Master's degree with one year of relevant experience.
