Join our dedicated team as a Case Manager Level II, where your passion for making a meaningful difference in the lives of children and families will be recognized. This remote full-time position is pivotal in delivering post-release services to children and their sponsors, emphasizing a strong foundation in trauma-informed care and child welfare practices. Your role will center around connecting families with vital local resources, facilitating family preservation, and ensuring the safety and well-being of children.

Key Responsibilities

  • Oversee a dynamic caseload of post-release services, managing Levels I & II.
  • Conduct thorough home study reports to evaluate risk, safety, and suitable child placement.
  • Partner with care providers to verify information and promote successful sponsorship.
  • Assess the sponsor’s capability to provide a safe and nurturing environment for the child.
  • Maintain consistent communication with children and sponsors through virtual and in-person visits.
  • Employ trauma-focused methodologies and brief therapeutic techniques to assist children and sponsors.
  • Deliver ongoing case management, including evaluations, planning, and psychoeducation.
  • Ensure accurate documentation while upholding the confidentiality of sensitive information.
  • Function as a mandated reporter, addressing any concerns related to abuse, neglect, or significant life occurrences.
  • Engage in training sessions and initiatives aimed at quality improvement.
  • Travel as necessary for in-person home studies and post-release visits, including occasional evenings and weekends.

Required Skills

  • Proficient understanding of family preservation, kinship care, trauma-informed practices, and child welfare policies.
  • Exceptional communication and interpersonal abilities to build rapport with diverse populations.
  • Competency in Microsoft Office and data entry systems.
  • Bilingual proficiency in English and Spanish (both verbal and written).
  • Empathetic, adaptable, and adept at forming connections with individuals from various backgrounds.
  • Strong problem-solving skills, with the ability to maintain composure under pressure.
  • Highly organized and detail-oriented, committed to fostering a positive impact.

Qualifications

  • A Bachelor's degree in Social Work (BSW) or a related field such as psychology, sociology, or behavioral science.
  • Minimum of 1 year of case management experience, particularly with immigrant populations, along with familiarity with trauma-informed clinical interventions for youth.
