Your role will center around connecting families with vital local resources, facilitating family preservation, and ensuring the safety and well-being of children.
Key Responsibilities
- Oversee a dynamic caseload of post-release services, managing Levels I & II.
- Conduct thorough home study reports to evaluate risk, safety, and suitable child placement.
- Partner with care providers to verify information and promote successful sponsorship.
- Assess the sponsor’s capability to provide a safe and nurturing environment for the child.
- Maintain consistent communication with children and sponsors through virtual and in-person visits.
- Employ trauma-focused methodologies and brief therapeutic techniques to assist children and sponsors.
- Deliver ongoing case management, including evaluations, planning, and psychoeducation.
- Ensure accurate documentation while upholding the confidentiality of sensitive information.
- Function as a mandated reporter, addressing any concerns related to abuse, neglect, or significant life occurrences.
- Engage in training sessions and initiatives aimed at quality improvement.
- Travel as necessary for in-person home studies and post-release visits, including occasional evenings and weekends.
- Proficient understanding of family preservation, kinship care, trauma-informed practices, and child welfare policies.
- Exceptional communication and interpersonal abilities to build rapport with diverse populations.
- Competency in Microsoft Office and data entry systems.
- Bilingual proficiency in English and Spanish (both verbal and written).
- Empathetic, adaptable, and adept at forming connections with individuals from various backgrounds.
- Strong problem-solving skills, with the ability to maintain composure under pressure.
- Highly organized and detail-oriented, committed to fostering a positive impact.
- A Bachelor's degree in Social Work (BSW) or a related field such as psychology, sociology, or behavioral science.
- Minimum of 1 year of case management experience, particularly with immigrant populations, along with familiarity with trauma-informed clinical interventions for youth.
