Position Overview
We are currently offering two distinct Case Manager roles:
- NOVA area: Concentrating on Behavioral Health, with a focus on the Serious Mental Illness (SMI) Team.
- Lynchburg area: Specializing in Long-Term Services and Supports (LTSS).
- Conduct comprehensive clinical assessments to evaluate eligibility for case management based on health and psychosocial factors.
- Collaborate with members, caregivers, physicians, and healthcare professionals to create and implement tailored care plans.
- Provide assessments through telephonic communication, face-to-face interactions, and home visits when required.
- Monitor and evaluate care plans, tracking progress and making necessary adjustments.
- Maintain an active caseload while engaging in proactive outreach and case management.
- Facilitate interdisciplinary care team meetings to ensure members receive coordinated services across behavioral and long-term care spectrums.
- Utilize motivational interviewing techniques to educate and support members.
- Identify barriers to care and coordinate essential resources and support.
- Conduct local travel for member visits (up to 40%), with mileage reimbursement included.
- Compassion and strong interpersonal abilities.
- Attention to detail combined with proficient computer skills.
- Capability to multitask effectively while documenting accurate case notes.
- Proficiency in motivational interviewing techniques.
- Strong aptitude for identifying barriers to care and coordinating necessary resources.
- Must be a resident of Virginia (NOVA or Lynchburg); other locations will not be considered.
- A Bachelor’s or Master's degree in social sciences, psychology, gerontology, public health, social work, or a related field, OR an active, unrestricted LVN/LPN license.
- Preference for candidates with a Bachelor's degree in Nursing.
- 1-3 years of experience in case management, disease management, managed care, or within a medical/behavioral health setting.
- A valid and unrestricted driver's license with a clean driving record.
- Reliable transportation for field visits is essential.
- A home office equipped with high-speed internet connectivity is required.
