Key Responsibilities

  • Oversee and manage Care Teams across various markets, ensuring adherence to high care standards.
  • Develop and refine operational systems to enhance service delivery and team performance.
  • Formulate and execute operating plans that align with organizational goals through cross-departmental collaboration.
  • Monitor and analyze key performance indicators (KPIs) and market metrics to evaluate team effectiveness and organizational performance.
  • Drive initiatives for continuous improvement, focusing on cost reduction, outcome enhancement, and member satisfaction.
  • Lead talent acquisition and development efforts to build mission-driven teams reflective of our core values.
  • Support Care Team Managers in managing remote, community-based teams and promoting a positive work environment.
  • Facilitate seamless communication between headquarters and care delivery teams while championing change management initiatives.
  • Integrate innovative technology solutions to optimize operations and enhance team productivity.

Qualifications

  • A Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or a related discipline is required; a Master's degree is preferred.
  • A minimum of 5 years of experience in managing high-performing, client-focused teams, particularly in settings addressing Social Determinants of Health.
  • Experience in healthcare-related fields, especially within Medicaid, Medicare, or population health management, is highly valued.
  • Demonstrated success in scaling front-line teams through effective recruitment, onboarding, and training.
  • A solid background in finance and operations, with measurable outcomes in recruitment, staff retention, and program performance.
