Key Responsibilities
- Collaborate with a multidisciplinary healthcare team to assess patient needs and optimize individualized care plans.
- Conduct thorough evaluations by reviewing medical histories, clinical data, and support requirements.
- Ensure continuous care coordination through regular follow-ups and necessary treatment adjustments.
- Partner with primary care providers, care coordinators, medical directors, and families to create tailored treatment and discharge strategies.
- Active RN or LPN license (North Carolina or Compact State required).
