Key Responsibilities
- Oversee a caseload of healthcare members requiring long-term care, prioritizing their overall health and well-being.
- Conduct monthly and quarterly assessments of members, with approximately 80% of responsibilities involving travel to members' residences.
- Create comprehensive care plans by evaluating the needs of members, caregivers, and healthcare providers, ensuring the appropriate services are recommended and provided.
- Ensure seamless service delivery by monitoring care provisions and following up through both in-person and telephonic check-ins.
- Collaborate closely with healthcare providers, discharge planners, and physicians to facilitate effective care coordination across acute and long-term care settings.
- Proactively address and resolve member complaints and appeals, ensuring that all members feel acknowledged and supported.
- Active RN license in North Carolina, in good standing.
- Proficient in using electronic medical records, along with strong skills in Microsoft Office and Teams.
- Exceptional communication and interpersonal skills.
- Strong organizational capabilities and meticulous attention to detail.
- Ability to travel for member visits as required.
- Bachelor's degree in Nursing or a related field.
- A minimum of 2 years of experience in Care Management or a related discipline.
