Position Overview

As a Home Health RN Case Manager, you will be responsible for delivering patient-centered care in a home environment. This role entails the independent management of a patient caseload, requiring advanced assessment, decision-making, and teaching competencies. You will collaborate with physicians, patients, families, and a multidisciplinary care team to ensure optimal patient outcomes.

Key Responsibilities

• Conduct thorough assessments of patients' physical, functional, and psychosocial conditions.
• Develop tailored care plans in conjunction with patients, families, and healthcare professionals.
• Deliver skilled nursing services, including medication administration, wound care, and chronic disease management.
• Educate patients and their families on self-care techniques and treatment plans to foster independence.
• Track patients' progress and adapt care plans as needed to achieve the best results.
• Collaborate with an interdisciplinary team to guarantee comprehensive, high-quality care.
• Maintain precise documentation in alignment with regulatory and organizational guidelines.

What We Are Looking For

• A valid RN license in the state of Wisconsin (mandatory).
• CPR Certification (mandatory).
• Bachelor's degree in Nursing (BSN) preferred.
• At least one year of relevant clinical nursing experience (medical-surgical preferred).
• Experience in home health as a case manager or primary nurse is preferred.
• Proficiency in home health medical record software (Epic Remote Client preferred).
• Strong skills in IV therapy, including knowledge of venous access devices and ambulatory pumps.
• A valid driver's license and reliable transportation for patient visits.
• Excellent communication, problem-solving, and patient education abilities.
