Key Responsibilities
- Conduct thorough assessments of patients' physical, functional, and psychosocial conditions.
- Develop tailored care plans in conjunction with patients, families, and healthcare professionals.
- Deliver skilled nursing services, including medication administration, wound care, and chronic disease management.
- Educate patients and their families on self-care techniques and treatment plans to foster independence.

- A valid driver's license and reliable transportation for patient visits.
- Excellent communication, problem-solving, and patient education abilities.
