Key Responsibilities
- Oversee a caseload of healthcare members requiring long-term care, ensuring they receive appropriate services and support.
- Conduct regular member interactions, which include both in-home visits and follow-up communications (80% travel).
- Execute assessments with members, caregivers, and healthcare providers to collect information for developing care plans.
- Monitor the execution of care services, identifying and addressing any deficiencies in service delivery.
- Authorize and coordinate referrals for additional services, working collaboratively with healthcare partners.
- Assist members, caregivers, and healthcare providers in resolving complaints and navigating appeals as necessary.
- Integrate informal or voluntary services into care plans, enhancing overall member support.
- Collaborate effectively with discharge planners, physicians, and other stakeholders to formulate comprehensive care and discharge strategies.
- Active RN License in North Carolina, in good standing.
- Bachelor's Degree in Nursing or a related discipline.
- Minimum of 2 years' experience in Care Management.
- Proficiency in electronic medical records and Microsoft Office Suite, including Microsoft Teams.
- Valid Driver's License in North Carolina.
- Exceptional communication skills, with the ability to work autonomously in the field.
- A minimum of 2 years' experience in Care Management is essential.
- Preferred: 2 years in Care Management or Service Coordination, ideally within a managed care framework.
- Preferred: Bilingual proficiency in English and Spanish.
