Key Responsibilities
- Deliver comprehensive case management for adult medical, surgical, and critical care patients.
- Adapt to various facilities as required to meet patient care demands.
- Ensure patient care plans are meticulously updated and executed in line with established best practices.
- A minimum of 2 years of experience in Case Management.
- Proficient in EPIC, SBAR, Microsoft Office, and possess strong keyboarding skills.
- Flexibility to float between healthcare facilities as necessary.
- Experience in handling adult medical/surgical/critical care cases.
- NICU/Maternity experience is advantageous.
- Certification in Case Management (ACM/CCM) is preferred.
- Current RN licensure in the state of North Carolina.
- Demonstrated experience in a healthcare environment with an average daily census of 750 patients.
- No call duty required, with weekend work expected every 6th weekend as necessary.
- A Bachelor's degree in Nursing is preferred.
