Key Responsibilities
- Patient Advocacy: Conduct comprehensive interviews and assessments of senior clients who face various challenges in social, emotional, financial, or health-related areas. Formulate actionable and tailored care plans based on these evaluations.
- Care Planning & Coordination: Assess complex cases and design individualized treatment plans that align with the unique needs of each client, ensuring successful implementation of strategies.
- Family Support: Collaborate closely with patients’ families, providing them with support through individual and group counseling, caregiver sessions, and respite care opportunities.

- Adaptability: Exhibit flexibility in addressing evolving priorities, regulations, and patient care demands.
- Experience: A minimum of one year of RN Case Manager experience in a hospital setting; three years of experience preferred.
- Education: Current Registered Nurse (RN) license required; BSN or MSW is preferred.
- Additional Experience: 3-5 years of experience in an acute care hospital, critical care, or emergency department setting.
- Certifications: CPR and ACLS certifications required.
- Flexibility: Capability to adapt to diverse patient care environments and flexible scheduling needs.
