Key Responsibilities
- Contribute to the development and execution of the Quality Improvement Program, fostering continuous enhancements in patient care and services.
- Aid in the implementation of approved recommendations and evaluate the effectiveness of corrective actions.
- Review complaints, errors, incidents, and grievances, ensuring proper documentation and resolution within set timelines.
- Assist in the formulation and enforcement of service policies as mandated by governing bodies.
- Maintain effective communication channels with leadership and stakeholders to ensure alignment with quality benchmarks.
- Facilitate staff development through orientation, ongoing education, and evaluations that incorporate Quality Improvement initiatives.
- Oversee organizational Performance Improvement activities to fulfill the agency's objectives.
- Coordinate risk management initiatives, ensuring guidelines are established to address cases involving risk factors such as infectious diseases.
- Conduct home visits to observe care, evaluate equipment and supplies, and gather feedback from patients and families regarding their satisfaction with care and services.
- Uphold patient confidentiality in accordance with applicable laws and regulations.
- Promote a compliant and secure workplace that adheres to federal, state, and local regulatory standards.
- Consult with staff as required to ensure the provision of safe, efficient, and high-quality care.
- Excellent verbal and written communication abilities.
- Strong capacity to maintain a positive attitude while navigating goal achievement.
- Proven experience in the development, implementation, and evaluation of continuous quality improvement programs.
- Must possess a valid Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) license.
- A minimum of 2 years of experience in home care is required.
- Demonstrated experience in advancing quality improvement initiatives within healthcare settings.
