Key Responsibilities
- Perform in-home health evaluations to assess both physical and mental health of patients.
- Create and execute personalized care plans tailored to the unique needs of each individual patient.
- Leverage UAS technology to effectively monitor and coordinate patient care.
- Provide education to patients and their families concerning self-care practices and medication management.
- Collaborate with multidisciplinary teams to ensure seamless and ongoing patient care.
- Compassionate and patient-focused nursing skills.
- Strong assessment and care planning expertise.
- Familiarity with UAS technology for patient care coordination.
- Excellent communication abilities for educating patients and their families.
- Collaborative mindset to work effectively with interdisciplinary teams.
- Valid New York State Registered Nurse license.
- A minimum of 2 years of nursing experience, with a preference for candidates from home care or community health environments.
- UAS certification is preferred; however, training can be provided for the right candidate.
