We're looking for a compassionate and dedicated RN or LPN to be a key part of our remote healthcare team. In this role, you'll connect with patients through telehealth calls and text messages to help them manage their chronic conditions and offer emotional support as they navigate their healthcare journey.
Key Responsibilities
Make outbound calls to new and returning patients to check in and provide guidance on their care
Engage with patients via text to answer questions, offer advice, and foster positive relationships
Monitor and track any changes in patients' symptoms or behaviors, ensuring timely updates
Collaborate with physicians and specialists to ensure coordinated and effective patient care
Educate patients on health maintenance, disease prevention, and managing chronic conditions
Assist patients in obtaining referrals to other healthcare providers or facilities when needed
Keep accurate and up-to-date patient medical records
Provide emotional support and offer friendly encouragement to patients and their families
Qualifications
Prior experience in nursing or a related medical field
Comfort with using medical software and telehealth equipment
Ability to build rapport with patients and provide exceptional care from a distance
Strong Problem-solving Abilities And Critical Thinking Skills
Ability to thrive in a dynamic, fast-paced environment
