Key Responsibilities
- Act as a trusted advocate for patients, ensuring their care requirements are addressed with empathy and precision.
- Provide CCM and RPM services while maintaining a comprehensive understanding of program benefits and guidelines.
- Accurately document all patient interactions in the Electronic Medical Record (EMR) system.
- Surpass targets for outbound calls, caseload management, and billable time expectations.
- Perform data entry and uphold meticulous documentation of patient communications.
- Ensure adherence to regulatory standards and organizational protocols.
- Assist in delivering and setting up RPM devices as needed.
- Manage pre-authorizations, referrals, prescription refill requests, and medication reconciliation.
- Coordinate durable medical equipment (DME) services and review hospital records to facilitate care planning.
- Collaborate with external providers and Primary Care Physicians (PCPs) to ensure comprehensive, coordinated care.
- Educate patients on prescriptions, disease management, and personalized lifestyle recommendations.
- Address patient queries, triage notes, and callback requests efficiently and professionally.
- Certification as a Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or Med Tech (or equivalent designation).
