Key Responsibilities
- Conduct comprehensive assessments to gain a deep understanding of each individual's clinical, psychosocial, and service preferences.
- Identify and address critical health and safety needs, including barriers to care stemming from social determinants of health.
- Collaborate effectively with an interdisciplinary team to integrate insights into thorough assessments and tailored Life Plans.
- Develop and maintain individualized Life Plans that encompass both clinical and non-clinical healthcare needs and services.
- Oversee the successful implementation of Life Plans, adjusting as necessary to accommodate evolving needs.
- Promote health and wellness opportunities that empower individuals to achieve optimal well-being.
- Coordinate access to chronic disease management resources and ensure comprehensive support.
- Assist individuals and their families in navigating clinical and community resource referrals, including transitional care planning and follow-up.
- Advocate for long-term care supports and services, ensuring a seamless coordination process.
- Ensure that all services are culturally and linguistically appropriate for diverse individuals and families.
- Encourage self-advocacy and promote the capacity for individuals to direct their own care.
- Utilize Health Information Technology (HIT) for accurate documentation and effective communication within the care coordination team.
- Maintain confidentiality and handle health records with the utmost integrity, adhering strictly to HIPAA regulations.
- Comply with all state and federal laws, Medicaid regulations, and organizational policies.
- Engage in ongoing professional development and training opportunities to enhance your skill set.
- Ability to respond quickly and effectively in crisis situations, ensuring the well-being of individuals.
- Proficient technology skills, including familiarity with managing health records.
- Strong understanding of ethical and professional boundaries and responsibilities.
- Proven reliability, time management skills, and independent problem-solving abilities.
- Bachelor’s degree in a relevant field with two years of relevant experience, OR
- Registered Nurse (RN) license with two years of relevant experience (including case management or service coordination), OR
- Master's degree with one year of relevant experience.
