Key Responsibilities
- Evaluate and organize data to assess member eligibility for the Population Health Management Program.
- Deliver timely, effective, and equitable care, adhering to established protocols to safeguard the health and safety of our members.
- Oversee case assignments, which include outreach, documentation, monitoring progress, and ensuring efficient case closures.
- Maintain precise and comprehensive reporting and documentation, fostering collaboration with departmental staff and clients.
- Assist members in overcoming challenges to improve their health by addressing social determinants, psychosocial issues, and motivational drivers.
- Educate members on healthcare navigation, empowering them to make informed choices that promote better health outcomes and cost-efficient care.
- Support operational aspects to fulfill organization requirements and enhance customer satisfaction.
- Uphold confidentiality across all medical records and data management systems.
- Participate in Quality Management/Utilization Management Committee meetings, handling preparations, minute-taking, data analysis, and reporting.
- Manage on-call and weekend shifts, as necessary.
- Stay informed on industry standards and best practices through continuous professional development and training.
- Undertake additional responsibilities as required by program changes.
- Active Illinois LCSW, LCPC, or LPC licensure.
- A minimum of five years' experience within healthcare settings or Managed Care Organizations.
- Fundamental understanding of case management principles and healthcare management practices, with a focus on motivational interviewing.
- Strong clinical judgment with exceptional verbal and written communication skills.
- Outstanding organizational, problem-solving, and time-management capabilities.
- Proven ability to manage multiple priorities and adapt to dynamic business needs.
- Self-sufficient work ethic, capable of handling fast-paced workflows in a remote environment.
- Proficiency in Microsoft Word, Excel, Access, Outlook, and experience with various electronic medical records (EMRs).
- Relevant licensure as specified (LCSW, LCPC, or LPC).
- An extensive background in healthcare or managed care settings, demonstrating experience in case management and healthcare system navigation.

- Salary: $70,000 per year, with compensation determined by factors such as experience, skills, location, and licensure.
- Flexible daytime hours available, with options for four 10-hour shifts per week, Monday to Friday between 6 AM and 6 PM CT, with flexibility after successful orientation.
