Key Responsibilities
- Assess data to establish member eligibility for Population Health Management programs.
- Deliver timely, effective, equitable, and member-focused care, following HMO procedures.
- Manage case assignments, including outreach, documentation, progress tracking, and case closure.
- Collaborate with colleagues and clients while maintaining documentation and reporting standards.
- Assist members in overcoming barriers to care by addressing psychosocial issues and social determinants of health.
- Educate members on navigating the healthcare system to ensure they receive high-quality, cost-effective care.
- Uphold confidentiality concerning all medical records and data.
- Participate in quality management and utilization management committee meetings, including material preparation and follow-up tasks.
- Provide off-hour/weekend call coverage as required.
- Stay updated with case management best practices and licensure requirements.
- Undertake additional responsibilities as necessary due to program changes.
- Current licensure in Illinois as an LCSW, LCPC, or LPC.
- A minimum of five years of experience in various healthcare environments or managed care organizations.
- Strong grasp of case management principles and healthcare management.
- Proficient in motivational interviewing and member engagement techniques.
- Exceptional clinical judgment and strong verbal and written communication abilities.
- Strong organizational, analytical, and time management skills.
- Capacity to work autonomously and handle multiple priorities in a fast-paced setting.
- Proficient in Microsoft Office Suite and experience with various Electronic Medical Record (EMR) systems.
- Must hold a valid Illinois license as an LCSW, LCPC, or LPC.
- At least five years of experience in diverse healthcare settings or managed care organizations.

Compensation And Benefits
- Annual salary: $70,000
- Flexible scheduling options following successful orientation and supervisor evaluation.
- Option for four 10-hour shifts.
- Work hours are Monday-Friday between 6:00 AM - 6:00 PM CT.
- Comprehensive benefits package including medical, dental, and vision insurance, a 401(k) plan with a 3% employer match, life and disability insurance options, and an Employee Assistance Program (EAP).
