Key Responsibilities
- Engage in data analysis to identify eligible members for the Population Health Management Program.
- Coordinate care to ensure timely and effective member-centric interventions.
- Manage case assignments from outreach to closure while ensuring meticulous documentation and progress tracking.
- Collaborate with team members and clients to meet reporting standards and participate in meetings.
- Support members in achieving wellness and health autonomy by addressing psychosocial issues and barriers.
- Educate members on navigating the healthcare system to promote quality and cost-effective interventions.
- Assist in the operational aspects of the division to meet customer requirements.
- Uphold strict confidentiality concerning all medical records and data.
- Participate in Quality & Utilization Management committee meetings, if applicable.
- Engage in off-hour/weekend call rotations as required.
- Continuously develop professional knowledge and skills in line with case management standards and licensure requirements.
- Undertake additional responsibilities as assigned, adapting to changes in program requirements.
- Current LCSW, LCPC, or LPC licensure in Illinois.
- A minimum of five years of experience in diverse healthcare settings or within a Managed Care Organization.
- Strong understanding of case management principles, healthcare management, and reimbursement components, coupled with experience in motivational interviewing.
- Excellent clinical judgment along with strong verbal and written communication skills.
- Exceptional organizational, problem-solving, and time management abilities.
- Proficiency in software applications such as MS Word, Excel, Access, PDF, and Outlook.
- Experience with multiple Electronic Medical Records (EMRs).
- Reliable high-speed internet connection, home office, and backup internet options.
- Relevant license (LCSW, LCPC, or LPC) in Illinois is required.
- A minimum of five years of experience in healthcare settings is essential.
- Strong clinical judgment and communication skills are necessary.
