Key Responsibilities

  • Lead, mentor, and develop a multidisciplinary care management team to optimize performance.
  • Assist in capacity planning, recruitment, and training of new team members.
  • Analyze team performance metrics, focusing on operational efficiency and clinical outcomes.
  • Collaborate with management to ensure adequate staffing and support for team members.
  • Provide clinical oversight for complex patient scenarios, guiding the team towards effective solutions.
  • Identify and implement process improvements to enhance quality of care.
  • Monitor key performance indicators (KPIs) to assess team success.
  • Serve as a subject matter expert in chronic care management, transitions of care, and remote patient monitoring.
  • Collaborate with cross-functional teams to drive innovation and improve efficiencies.

Required Skills

  • Registered Nurse (RN) with a compact license (required).
  • Strong leadership skills with a minimum of 2-3 years of experience managing clinical teams.
  • Proficiency in managing complex projects within a remote team setting.
  • Familiarity with managed care frameworks in New York State is essential.
  • Excellent communication and collaboration skills.
  • Detail-oriented with the ability to adapt to shifting priorities.
  • A self-starter with an enthusiasm for integrating new technology into healthcare practices.

Qualifications

  • Registered Nurse with a New York State nursing license (preferred).
  • Minimum of 4 years nursing experience in acute care settings (required).
  • Bilingual in English and Spanish (preferred).
