Key Responsibilities
- Manage a caseload of patients with an emphasis on bridging gaps in both clinical and non-clinical care.
- Identify and connect patients with community resources to support their healthcare needs.
- Educate patients to enhance their health literacy and assist them in effectively managing their chronic conditions.
- Oversee medication management, ensuring proper reconciliation, adherence, and coordination of refills while identifying potential medication issues.
- Ensure the timely provision of services, including Home Health, Durable Medical Equipment (DME), Home Infusion, and other vital needs.
- Address patient inquiries and cultivate an open dialogue to fully understand their needs.
- Assist in managing referrals and scheduling appointments.
- Active LPN (Compact or Multi-State) license.
- Proficiency in Microsoft Office Suite.
- Exceptional written and verbal communication abilities.
- Experience coordinating care and managing patient caseloads.
- Familiarity with Epic is preferred.
- Bilingual candidates are a plus.
- Additional single-state licensures are advantageous.

- Pay: \(21/hr - \)28/hr, with opportunities for overtime to enhance earnings.

- This is a full-time, Monday through Friday role with no weekend shifts and minimal on-call rotation (once per year on average).
