Key Responsibilities
- Manage a patient caseload, addressing clinical and non-clinical care gaps.
- Connect patients with community resources tailored to their healthcare needs.
- Educate patients on effectively managing chronic conditions and enhancing health literacy.
- Support medication management, which includes reconciliation, adherence, and refill coordination.
- Ensure timely access to services such as Home Health, DME, and Home Infusion.
- Serve as an accessible point of contact for patients, facilitating communication and answering inquiries.
- Assist in scheduling appointments and managing referrals.
- Active Multi-State/Compact License (LPN/LVN/NCL).
- Proficient in Microsoft Office Suite.
- Excellent written and verbal communication abilities.
- Active Nursing License.
- Experience in chronic care management is preferred.
- Prior experience in a healthcare or clinical environment is beneficial.
