Key Responsibilities
- Manage a patient caseload with a focus on bridging gaps in both clinical and non-clinical care, ensuring a resolution-oriented approach.
- Coordinate resources by identifying and connecting patients to essential community services that enhance their care and quality of life.
- Educate patients by delivering clear and actionable information on managing chronic conditions, fostering health literacy and self-management.
- Oversee medication management tasks, including reconciliation, adherence, and refills while addressing potential medication issues.
- Ensure the timely provision of critical services such as home health, durable medical equipment (DME), and home infusion.
- Advocate for patients by responding to inquiries and concerns, establishing open communication, and nurturing strong relationships.
- Assist with or manage referrals and appointment scheduling to enhance patient care.
- Active Multi-State/Compact License (LPN/LVN) (NLC).
- Proficiency in Microsoft Office Suite along with a strong technical aptitude.
- Outstanding written and verbal communication capabilities.
- Ability to operate independently in a remote setting.
- Reliable high-speed Wi-Fi and a dedicated, HIPAA-compliant workspace at home.
- Capacity to sit for extended durations, articulate clearly, listen attentively, and take thorough notes during phone interactions.
- Experience with Epic electronic health record (EHR) systems is a plus.
- Bilingual candidates are encouraged to apply.
