Key Responsibilities
- Oversee a designated panel of patients, proactively identifying and addressing gaps in both clinical and non-clinical care.
- Connect patients with relevant community resources to meet their healthcare needs.
- Educate patients on chronic disease management and enhance health literacy.
- Implement medication management practices, including reconciliation, adherence support, and refill coordination.
- Ensure timely access to essential services such as Home Health, Durable Medical Equipment (DME), Home Infusion, and other patient necessities.
- Engage patients effectively to address concerns, respond to inquiries, and promote open communication.
- Assist with referrals and appointment scheduling to facilitate seamless care coordination.
- Active Multi-State/Compact License (LPN) (NLC) (LVN)
- Proficient technical skills, including expertise in Microsoft Office Suite
- Strong verbal and written communication abilities
- Experience with Epic EMR is preferred
- Bilingual candidates are a plus
- Additional state LPN licensure is advantageous
