Key Responsibilities
- Conduct comprehensive assessments of patients' medical, emotional, and social requirements.
- Collaborate with healthcare professionals to develop and implement tailored care plans.
- Provide education to patients on managing chronic conditions and encourage adherence to prescribed treatments.
- Facilitate seamless communication as a liaison among patients, caregivers, and the healthcare team.
- Monitor data from RPM devices identifying potential health risks and channel findings to the care team as necessary.
- Maintain precise documentation of patient interactions and ensure compliance with healthcare regulations.
- Assist in enrolling eligible patients into CCM and RPM programs, evaluating program effectiveness, and suggesting improvements.
- Proficient in electronic health records (EHR) systems.
- Familiarity with RPM devices and data monitoring tools is advantageous.
- Strong communication and interpersonal abilities, adept at educating and motivating patients.
- Excellent organizational skills and the ability to manage multiple tasks with attention to detail.
- Must be a Certified Medical Assistant (CMA).
- Certification in disease management, case management, or patient education is a plus.
- Minimum of 2 years of experience in patient care, chronic disease management, or care coordination.
- Experience with CCM or RPM programs is highly desirable.
