Key Responsibilities
- Oversee patient care, ensuring comprehensive support for individuals with chronic conditions while addressing both clinical and non-clinical care gaps.
- Facilitate connections for patients by identifying and coordinating access to essential community resources.
- Empower patients through health education and guidance to enhance their ability to manage their conditions effectively.
- Support medication management by reconciling prescriptions, ensuring adherence, identifying issues, and organizing refills.
- Assist in the timely coordination of essential health services such as home health, durable medical equipment (DME), and home infusion.
- Engage with patients to address concerns, fostering an environment of open communication that enhances their care experience.
- Aid in the management of referrals and appointments to ensure seamless continuity of care.
- Possession of an active Multi-State/Compact LPN or LVN License (NLC).
- Proficiency in digital tools, including the Microsoft Office Suite.
- Exceptional communication abilities, both written and verbal.
- Prior experience in chronic care management or a related field is preferred.
- Familiarity with Epic is advantageous.
- Bilingual candidates are encouraged to apply.
- Additional single-state LPN licenses are a plus.
