Key Responsibilities
As a Nurse Practitioner, you will:
- Conduct comprehensive assessments, including annual wellness exams, both in patients’ residences and virtually.
- Provide tailored education and counseling to patients and their families, empowering them to manage their health conditions effectively.
- Maintain accurate and current patient records to ensure seamless communication and continuity of care.
- Collaborate with primary care providers, specialists, and ancillary services to deliver holistic patient care.
- Administer evidence-based care to reduce hospitalizations and enhance patient satisfaction.
- Prescribe medications, order diagnostic tests, and work collaboratively with physicians to develop impactful treatment plans.

The ideal candidate will possess:
- Bilingual proficiency in English and Spanish (highly preferred).
- Proficiency in using medical instruments and electronic health records (EHR) systems.
- Strong verbal and written communication skills to facilitate effective interactions with patients and healthcare professionals.
- A solid understanding of federal and state regulations, including HIPAA and OSHA requirements.
Candidates must have:
- An active and unrestricted Registered Nurse and Nurse Practitioner (NP) or Physician Assistant (PA) license.
- Board certification relevant to your licensure (NP: ANCC/AANP; PA: NCCPA).
- A current and unrestricted DEA certificate.
- A minimum of 2 years of direct patient care experience, preferably in managed care or health plan settings.
