Key Responsibilities
- Strategically plan, coordinate, and supervise healthcare services and training programs to achieve optimal outcomes, while working in tandem with the Operations management team.
- Guide and mentor clinical staff to foster professional growth and monitor performance effectively.
- Proactively identify and mitigate potential risks to elevate the quality of care delivered.
- Engage in Interdisciplinary team meetings to contribute to comprehensive patient care plans.
- Ensure adherence to company policies, state regulations, and compliance with Medicaid and Medicare standards.
- Lead case conferences and client-related discussions to assess care plans and patient progress.
- Chair various committees, including those focused on safety, infection control, and human rights, to uphold best practices.
- Serve as a vital link between nursing and care staff, families, guardians, and community medical providers to promote seamless communication.
- Oversee initial assessments and home visits for new healthcare contracts to ensure a smooth onboarding process.
- Supervise the implementation and evaluation of care plans, analyzing outcomes, medical records, and physician orders.
- Advocate for and promote services within your designated area to expand our reach and enhance our impact.
- Perform additional duties as required to support the team and ensure the provision of quality care.
- Strong communication capabilities with a proven ability to manage multiple tasks efficiently.
- Proficient knowledge of local, state, and federal funding sources and their respective regulations.
- Capacity to mentor and supervise clinical staff effectively.
- Willingness to travel within the assigned area as needed.
- Active Registered Nurse (RN) license from an accredited nursing program.
- Bachelor's Degree in Nursing, Social Services, or a related healthcare field.
- Demonstrated experience in healthcare management across diverse age groups.
