Key Responsibilities
- Conduct regular meetings with assigned cases and stakeholders to evaluate needs and develop tailored support plans.

- Address psychosocial challenges and guide participants to improve engagement and self-sufficiency.
- Collaborate with participants to create, implement, and monitor individualized service plans.
- Advocate on behalf of participants to secure benefits such as income support and entitlement programs.
- Oversee treatment plans to ensure objectives are achieved within established timelines.
- Offer financial assistance and budget management support when necessary.
- Complete case documentation and reports with accuracy and timeliness.

- A Bachelor’s degree in Social Work, Psychology, or a related discipline, accompanied by three years of experience in case management or counseling, or a Master's degree in a related field with one year of experience.
- A valid Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) certification is required.
