- Evaluate and devise comprehensive care plans for patients with intricate medical requirements.
- Facilitate seamless transitions of care between inpatient and outpatient environments.
- Collaborate with multidisciplinary healthcare teams to ensure exemplary patient outcomes.
- Act as an advocate for patients and their families, guaranteeing access to essential resources.
- Engage in quality enhancement initiatives and contribute to the overall effectiveness of the care team.
- Exceptional communication, interpersonal, and organizational capabilities.
- Strong patient advocacy and adept problem-solving abilities.
- Capacity to collaborate effectively with healthcare professionals.
- Bachelor of Science in Nursing (BSN); a Master’s degree in a related discipline is preferred.
- A minimum of 5 years of nursing experience, particularly in pediatrics is highly desirable.
- Certification as a Certified Case Manager (CCM) is preferred, or willingness to obtain it within two years of employment.
- Experience in Case Management (Discharge Planning and/or Utilization Management) is preferred.
