- Collaborate with therapists, nurses, and various healthcare professionals to create and execute comprehensive care plans.
- Evaluate patient needs and oversee medical conditions, including pain management and post-operative care.
- Inform patients and their families about conditions and treatment options.
- Engage in interdisciplinary team meetings to support the overall well-being of patients.
- Valid Physician Assistant (PA) license in New Hampshire.
- Proficient clinical skills with a strong focus on patient-centered care.
- Excellent communication and interpersonal abilities.
- A commitment to improving the lives of older adults.
- Completion of an accredited Physician Assistant program.
- Experience in a skilled nursing facility or post-acute care setting is preferred.
