Key Responsibilities
- Design and execute customized care plans tailored to the unique needs of each member throughout the care continuum.
- Perform clinical assessments via telephonic and in-person methods to identify, assess, and address member requirements.
- Coordinate care for high-risk patients, including those with chronic ailments or disabilities.
- Oversee care management to optimize the effective and efficient use of health benefits, focusing on gap-in-care management.
- Facilitate authorizations, referrals, and the execution of care plans.
- Collaborate with Medical Directors, Physician Advisors, and Interdisciplinary Teams to evaluate treatment strategies.
- Ensure adherence to all relevant regulations, policies, and procedures.
- Assist in resolving issues related to providers, claims, or services.
- Valid and unrestricted Registered Nurse (RN) license, with a Compact/Multi-State license required.
- Bachelor of Science in Nursing (BSN) is preferred.
- Proven nursing experience, particularly in case management, care coordination, or patient advocacy.
- Familiarity with medical/clinical cases concerning pregnant and postpartum women.
- Experience in maternity care (Labor & Delivery, Postpartum, Mother/Baby) is advantageous.
- Solid understanding of physical, psychological, socio-cultural, and cognitive patient needs.
- Strong communication, problem-solving, and analytical abilities.
- Capacity to operate both independently and as part of a team in a fast-paced remote environment.
- Active and unrestricted Registered Nurse (RN) license is mandatory.
- Experience in nursing, particularly with a focus on case management and care coordination, is essential.
